CPT® 99496

CPT® 99496 billed with CPT® 99490 on same date of service with 25 modifier each. Medicare denied the claim for CPT® 99496 as modifier combination is invalid. We also tried removing the modifier 25 from 99496 but it is still denied for same reason. Is it correct coding?
