Q0091, P3001, 99214 billing

Hi,

One of our clinic physician billed the code Q0091, P3001 & 99214 with Dx code V72.31 to a Commercial payer. Payer denied the codes Q0091 & 99214 incidental to another procedure & paid P3001 which is the lower billed amount.

Can anyone help us on this issue?
