Hello,

Office setting: I have my NP see a patient, do the Hx, exam, and assessment with plan. Then I see the patient briefly at the end and summarize the same treatment plan. My question is what "catch phrase" can I insert into the NPs EMR note so that I can still bill Incident To for that visit

Inpatient Hospital Setting: same question.

In the office, this is not incident to. The NP must bill for the service.
In the hospital (under Medicare) could satisfy "shared visit".
