Office visit

Im new at this but when a patient comes in for say: htn, diabetes, shoulder pain and asthma..dr performs a pFt...on e/m code, would I have all four codes then for the pFt to use dx code asthma? So all four dx on line E/M and just the asthma for the PFT?

Yes, I agree with rphelps.

The important thing is to link the diagnosis for a test to the correct indication. For an E/M service, all of the diagnoses support the medical necessity for visit. But some services, like diagnostic tests, require a diagnosis that supports the reason the test was done.
