Well Exam and Sick Visit on same day with ICD-10

Does anyone know if a child is seen for a well visit exam and the physician finds something wrong with the child such as sore throat, ear infection, fever,ect, if we will be able to still code a sick and well for ICD- 10. Would you select the Z00.121 abnormal findings DX code for this or just code the regular Z00.19 Normal exam and then list the code for the symptom's associated to the illness, regardless. Physician states the exam was abnormal because they found this issues , but the list only covers R70-R94 and this is for abnormal findings on examination of blood without dx. Any help would be greatly appreciated
