Chemo infusions/pheresis

When peds come in for chemo; the facility drops its charge. Our PedsHemoc docs
always check their patients before, during, and after the chemotherapy. Our peds nephrologists do the same with their patients undergoing pheresis. Their documentation is generally an E&M level of a 2 or 3. A consultant came in and said that they can be bill E&M services; especially since they are under a different TIN than the facility. I am more concerned about the peds nephrologist, because the majority of their patients are Medicare. Anyone have any experience with this?
