Ob nurse visit

Please help me, I'm so confused!! We recently started having our patients with a confirmed pregnancy (HPT) come in to the office to see our RN to go over her health history, prenatal history etc...My question is, do I bill for this visit? The RN does start the antepartum record when the patient comes to see her. Next question is.....when the patient comes in to see the doctor for her pap/cultures and actual first clinical visit, am I able to bill that as well or is that considered global? If I can bill, what codes should I use? Do I bill for the pap? What if she has had a pap in the last year and insurance won't pay for it? Sorry to be so complicated but this is really confusing me and I want to do it right! Thank You!
