Heart catheterization

Hi,

Can anyone give me coding for heart catheterization done in inpatient/outpatient setting with appropriate modifier to be use.

1. Left heart catheterization with ventriculography (inpatient/outpatient)
2. Left heart catheterization with coronary angiography (inpatient/outpatient)
3. Left heart catheterization with PTCA (inpatient/outpatient)
4. Left heart catheterization with PTCA and coronary stent (inpatient/outpatient)
5. Right heart catheterization with ventriculography (inpatient/outpatient)
6. Right heart catheterization with coronary angiography (inpatient/outpatient)
7. Combined right and left heart catheterization with ventriculography (inpatient/outpatient)
8. Combined right and left heart catheterization with coronary angiography (inpatient/outpatient)
