circumcision with release of ventral chordee

Please help

35yr old patient was diagnosed for phimosis and adhesions

procedure indicates that circumcision and release of ventral chordee were done.
No info on adhesions in the description.

i am confused in choosing CPT® for release of ventral chordee as the description indicates that using a knife, ventral chordee is released.

As per CPT®, service descriptions of release of chordee are different.
