billing for discharge services

Can I bill 99309 and 99316 together for a discharge visit with a patient in a SNF that lasted over an hour? (I am an NP)

the 99309 is a subsequent care per day and cannot be billed with the discharge..the 99316 is a discharge and it is anything over 30 minutes if it becomes more than 1 hour then use 99356. the 99356 is prolonged physician service in the inpatient setting requiring time beyond the usual service. then for each additional 30 minutes list separately in addition to code for prolonged physician service a 99357.

Remember that for Medicare, prolonged services need start and stop time, and are for face-to-face services.
