femoris musculocutaneus advancement flap

I need help with this please.


OP Nte says: left ischial decubitus ulcer was excised at the level of skin,subQ soft tissue and muscle, excised to punctate bleeding and viable subq soft tissue. posterior thight V-Y advancement flap incision was made incision was taken thru
skin and subq tissue to level of long head of the biceps femoris and the semitendinosus muscles. the semitendinosus and long head of the biceps femoris muscles were elevated and dissected out from deepest aspect adjacent to the sciatic nerve and vascular bundle. The overlying sking paddle was left attached. Once the muscles were dissected from their deepest aspect they were divided at their orgin and insertion proximally and distally. once good mobility performed the flat was advanced into the left ischial ulcer decect. The flap was secured and drain placed. The remaing open portion of the biceps femoris and semitendinosus muscle flap defect was dressed with vac dressing as well as the open portion of the partially complex closed ulcer defect.
