Development Screening

Need help coding for developmental screenings.
Teacher fills out a questionaire relating to ADHD. There is no formal scoring system. Based on the answers the physician determines the behavior is consistent with ADHD. Do you code this 96110?

Physician does Ages & Stages form on all ages of his patients. I can only find information from the AAP that indicates this test is only recommended for ages over 1 month to 60 months. Based in this information it doesn't appear that medical necessity for this particular test is met for older children. When there are behavioral issues in question wouldn't BASC-II or Vanderbuilt Rating be more appropriate?
