Billing Physical, Office visit, and Immunizations

What modifiers would I use the bill for a routine physical, office level, and immunizations? I have billed modifier 25 on the office visit, but the insurances(Premera) denies the Physical and pays the office visit and administration code for the immunization.

Most insurances will not pay for a physical and an office visit on the same day. We are allowed to bill them together, but it's to the insurance company's discretion as to whether they allow it or not. I advised my doctor to tell patients who come in for a physical to reschedule the physical for another day if the visit turns into diagnostic. We just explain to the patient that their insurance will not pay for both visits on the same day.

I always get paid for both by putting a 25 modifier on the office visit. Are you using V70.0 as the diagnosis for the physical?

Wow. I seriously only have two insurances who have ever paid for both. I use a 25 modifier for the E/M and V70.0 for the preventative dx. Interesting.

You will need to use a 25 on the Office visit as well as the preventative visit
