bundling

At least 90% of the time EOBs come back with CPT® code 21355 (Open Nose Reduction) bundled with 30520 (Repair of Nasal Septum). I am somewhat new at coding but all the material I have read says that those two codes should not be bundled. I would surely appreciate any help with this problem.

As long as you have a medically necessary diagnosis for each, shouldn't be an issue.

icd 9 code 7540 (congenital musculoskeletal deformities of skull, face, and jaw) is used for CPT® code 21335 and for 30520 icd 9 code 470 (deviated nasal septum). is this okay?

Should be OK. Guess you have to send documentation of medical necessity

No, in looking at this again you are saying you are treating fractured septum. Also coding 30520, repair of nasal septum. I can see why they denied 21335. Did you mean to say code 21335?

yes, it is 21335 with diagnosis 7540.

How is a fractured septum a congenital condition? What is the patient's age? I agree that both would not be coded together.

I so agree with you - I went to my supervisor and pointed out the diagnosis - it has been changed and now I will refile and see what happens. Thanks so much for your help.
