Definition of "Medical Direction" by a Physician

I am trying to figure out whether the QX modifier can be used for a NON-CRNA advanced practice nurse for anesthesia code 01996. I am trying to get a legitimate reference for the "medical direction" requirement. Is an order from a physician (without direct supervision) sufficient for the "qualified nonphysician anesthetist" to perform and bill for daily monitoring of a pain catheter?
