Protime 85610QW and Medicare

All of my profees are being paid by Medicare but the 85610 is not. I have always been paid in the past with the right dx along with 99211 and 85610QW but Medicare has quit paying for the 85610 line item. HELP!!!!!

I bill the Nursing Visit and the Protime together. Up until mid 2012 they flew through
MCR is splitting the line items and the Protime is now simply falling off the grid. No rejection ode no nothing.

The 99211 should not be billed unless there is an abnormal result and doctor changes order
