sentinel node biopsy

My issue is with cpt code 78195 and what it describes. My report has the surgeon injecting behind the areola with a methylene blue mixture...after making an incision and checking clip placement, "the area of the sentinel node was then identified using the Neoprobe". 2 additional nodes were found using the Neoprobe and excised. I am clueless on coding this. Can only one excision be coded?

Sentinel node

Look at 19290 thru 19295. For you code if your Surgeon placed the wire or clip. For the excisions look at 19125 and 19126 for methylene blue mixture...look at 38792. Without reading the actual op report I hesitant to code specifics. I like to read the report. But hope this helps.

"an elliptical incision was made radially in the lower inner quadrant. The thick part of the wire was already evident through the skin. The dissection was carried down to the pectoralis fascia, and after the tissue was excised, margin markers were placed on it, and it was sent to mammography where the clip was noted to be in the center of the tissue removed. The area of the sentinel node was identified using the Neoprobe. There was negligible radioactivity behind the sternum or in the supraclavicular fossa. The incision was then made near the bottom of the axillary hairline. Subcutaneous tissue was divided with cautery and a node was identified which was faintly blue in color. This was radioactive and excised. Two other nodes also showed radioactivity via Neoprobe and were excised."

what does the statement "the thick part of the wire was evident throught the skin" imply? That the surgeon was in the targetted area?
when "the clip was noted to be..." is that also confirming the correct location?
Can three excisions of sentinel nodes be coded?
