KENALOG Injection

Hi,
Good morning..
Hope somebody could help me to find out what should the correct CPT® code to be used.

If the PCP did the procedure:Kenalog injection:
shoulder 40 mg of kenalog was injected with into bicepital groove of left shoulder after 5 ml of 1% lidocaine and 5ml of 0.5% marcaine was injected first. Skin was sterilized first using betadine then wiping with alcohol. Ethyl chloride was topically sprayed immediately prior to needle penetration . Patient tolerated it well without complications..

And the dx code was 726.10

Can I use 99213, 20610, J3301 And A4211. But I'm not so sure about A4211 and I dont think they allowed this code to be get paid..

99213-25, 20610, j3301...Most insurances will bundle the a4211

So I can use 99213-25.
And for 20310, J3301 --will need to use A4211. Hard time understanding. Pls i need more explanation I mean pls elaborate. Thanks

Bill 99213-25, 20610, J3301 and do not bill A4211 simply because most payers will bundle (not pay) for code A4211.
