How to bill for a depo shot?

I need to bill for a depo shot, in addition to office visit. Should I put a modifier 25 for the office visit and bill for the shot and administration on separate lines? Also do I need the NDC code for the drug?

Yes: mod 25 on the E&M. NDC will depend on the payer's requirements. If it is Medicaid, possibly. Commercial insurances should recognize the appropriate HCPCS code for the drug.
