Is a keloid considered a lesion or a benign tumor?

the type of 'keloid' I know about is neither lesion, nor benign tumor, . . . rather is hypertrophic scar tissue that grows beyond the bounds of the original wound - like a scar "on steroids", if you will.

I agree it's scar tissue. If you are looking for the code for keloid injections, it's called intralesional injections CPT® code 11900.
