proc code 87804 modifier

When billing proc code 87804 to we place modifier 59 next to this code or the ov code? Are we allowed to use the modifier twice if we are billing for A & B?

Modifier 59 is never put on an E/M code.
I believe this question (A/B) has already been addressed in this forum.
