Advanced Directive Counselling

How do I code for time spent with patients counselling the various forms of advanced medical directive (medical proxy, living will, durable power of attorney, etc.). This is a presentation I give my patients, including the Power Point presentation I also give to my primary care students, as wellas handouts from the state health department on the subject, and answer questions they may have. The time usually takes about a full hour. This is separate from the "wellness visit", as most have already had their annual wellness visits, and that usually covers active medical problems, and advanced planning is too important and sensitive to be subsumed with other confounding matters. What is the ICD-9M and what is the CPT® or HCPS code to use?
