Implanon Insertion and Removal Question

Implanon was inserted into a patient's arm. The ultrasound showed it was inserted incorrectly, so it was removed. This all happened on the same day. How should this case be billed?

The following codes can be used when inserting and removing contraceptive implants:
Diagnosis code:
V25.50 Encounter for contraceptive management, insertion of implantable subdermal contraceptive
V25.43 Encounter for contraceptive management, surveillance of previously prescribed contraceptive methods (checking, reinsertion or removal of contraceptive device), implantable subdermal contraceptive.

CPT® codes (same for Implanon or Nexplanon):

11981 Insertion, non-biodegradable drug delivery implant

11982 Removal, non-biodegradable drug delivery implant

11983 Removal, with reinsertion, non-biodegradable drug delivery implant
