I am trying to find the J-code for billing the Depo subQ Provera 104.

Clarification: J1055 is contraceptive. J1056 is medroxyprogesterone acetate/estradiol cypionate 5 mg/25 mg

Thanks for the clarification, but one of my physicians has heard about Depo subQ Provera 104, his understanding is it is a different product than the J1051 and J1055. I researched Pfizer's website, it lists the subQ seperately and gives alot of information except the J code.
