CPT® Codes 92982,92984

Hi, I need help in solving this mystery (denial) for CPT® codes 92982, 92984. I have done billing for a patient with these codes (92982,92984,93510-26) in seventh month of 2010 and the insurance is texas medicaid. Firstly i have done all these three codes on a same claim. I got rejection from texas medicaid stating "procedure code is invalid or the combnation of procedure code and type of service is invalid" for CPT® codes 92982 and 92984. Then i separated CPT® code 93510-26 from 92982 and 92984, i got paid for CPT® code 93510-26. Problem continued with the CPT® codes 92982 and 92984. ECP is not accepting these procedure codes and continuously rejecting stating above mentioned reason. I sent the claim that contains CPT® codes 92982 and 92984 through paper. Surprisingly i got denial from texas medicaid stating the same reason that ECP mentioned in rejection "procedure code is invalid or the combination of procedure code and type of service is invalid" and one more thing that is surprising me is, until now i didn't receive a timely filing denial from texas medicaid. Because the time limit for texas medicaid is 90 days only. I'm fed up with these codes and i have to do something so that i get payment for these CPT® codes. So Please can anybody help me out from these CPT® codes. I'm also confused with the rejection and denial reasons. CPT® code 92982 is a primary code and 92984 is an add-on-code. I need to know where I am going wrong. Any help would be appreciated. Thanks in advance....

For dates of service 2010--the codes you mention are a left heart cath and PTCA. It would be necessary to see the procedure note to assign accurate codes. If a left heart cath was performed 93510-26 is accurate and if PTCA was also performed you need to specify the vessel(s) with LC, LD, or RC modifier. Also, if injections were done and interp. for cath was done, you need additional codes. Medicaid is asking for Type of service, where was the service performed. TOS "1" indicates professional service
