dialysis and subsequent hospital visit

Can you bill to Medicare a subsequent hospital visit 99232 or 99233 with modifier 25 and a dialysis code 90945 or 90935?


Does Medicare pay for a hospital discharge and admission to nursing home on the same day and if so how do you code? I don't believe they pay for both on same day....
