Billing more than 8 diagnosis codes for one visit

How do you bill more than 8 diagnosis codes for one visit ?

what prodedure code do you use eg,. 99214 for the first 8 codes then 99213 for the next codes and do you put in a modifier?

You cannot bill more than one established visit per day. Why do you want to bill 8 diagnosis codes when most insurances only look at the first one and most claims can only accomodate four.

Medisoft can accomodate 8 at a time. The reason why we bill more that 8 at times is that we have alot of capitated payers and so we have to bill everything for the vists.....dealing with alot of MRA coding. eg. when the md document diabetes with neuropathy that is billed 250.00, 250.60, 357.2. I usually bill 99213 with the first set of 8 diagnosis and then 99212 if more than 8. Is that correct billing ?

You can not add an additional office visit just because there are more than 8 diagnosis codes. You can see if your software vendor can accomodate more than 8 lines for this area of your software.
