If the physician is charging for smoking cessation counseling (CPT 99406) and also
an E/M for follow up on a chronic problem wouldn't they have to have two notes on
that day to document both services? In other words, you should not incorporate the smoking cessation into the E/M note correct?

There's no written requirement for a second note. however, since the smoking cessation codes are time based, time must be documented in the medical record. The billing provider should write, "I spent an additional XX minutes in discussing smoking cessation. The patient agreed to try Chantix....."
The E/M service billed needs to meet the requirements for that level of service, of course.
