Billing PT and nursing for Home Health Wound care

Is it possible to bill for both PT and nursing with a home health patient that is just getting skilled nursing for wound care. PT's are able to wound care but with this patient only nursing was ordered. The PT has gone in twice and done the wound assessment with vitals and the dressing change. We just don't know how to bill that visit. Technically a PT note can't be performed since there wasn't a PT evaluation done initially. Is there a PT wound assessment note that can be done?? Any help or hints would be appreciated.
