Dr did office visit and excisional biopsy of vaginal cyst x 2,(both were less than .05cm), would the proper procedure code be 49203 or 11420 and would you list the code twice or use a modifier?

Going on just the information given...I would prob go with 11420, and 11420-59...also take a look at 57135...
