Does CPT 77470 have to be billed at the same time CPT 77263 is submitted or can it be billed after treatment planning has already begun and then extra planning is involved? Can it be billed before treatment is completed? What are the documentation requirements for submitting CPT 77470 and CPT 77263?

77470- Billed once during course of treatment. Can be billed in addition to weekly management CPT code. A brief written statement of medical necessity (ex, heavy particles) showing time consuming or complex treatment procedure is fine.

77263 - Written treatment plan,, location, extent, volume of tumor(s) to be treated and critical structures surrounding them. May involve ordering special tests/procedures or placement of markers. Time/dose considerations, ports and devices. See CPT code description.
