Can we bill a consult E&M when a new Medicare patient is referred to us by a physical therapist? The therapist does have his own NPI.

That may depend on what state you are in. In Texas, a physical therapist cannot be the referring provider for any service. Does your state allow a physical therapist to be a referring provider?
