Facet Joint Injection/modifier

For a Facet Joint Injection at one location would I link modifier 25 for the OV and use 50 for the injection?

Your office visit would have a 25 modifier if are able to bill for the OV. If the facet injection is done bilaterally you would add the 50 modifier

Facet Joint Injection/modifier

You would not use modifier 25 unless there was a separatly identifiable service rendered. You would not use modifier 50 unless he injected bilateraly.
