My dr. Did a procedure on a patient using proc code 11042 and dx code 682.5 three days later he proformed another procedure, proc. Code 12032 using modifiers 58 stage and 78 return to the or. With the same dx code 682.5. However caresource does not accept these modifiers under Medicaid guide line.

they paid for the first proc. 11042 but denied the second proc. 12032 even though I rebill it using modifer 76. We did get authorization from caresource to performed the procedures.

can anyone help me by letting me know how can bill the proc. Code 12032 to get it paid. I hope you are able to understand what I am saying.
