Completion thyroid surgery

I wrestle with the notation under cpt 60260, "For bilateral procedure, report 60260 with modifier 50". Does this mean, completion thyroidectomy on one lobe; if there is resection of remaining tissue on both lobes, append modifier -50. If the patient has had a prior right total lobectomy and now a left total lobectomy, should we be coding 60220 ? My research has not been very successful.

You code what was done and dictated. Code 60220 states a total thyroid lobectomy, on one side.
Code 60260 states removal of remaining thyroid tissue after a surgery where only a portion was removed in lobe. So, this is more extensive than the first procedure same lobe. If in global may be a staged procedure (58) on second code.
