anterior exoneration

How do you code anterior exoneration if the patient has already had a TAH/BSO years ago? The dr. removed the bladder, ureters, ovarian pedicles, urethra, bilateral pelvic and common iliac lymph nodes. He calls it "anterior exoneration with ileal loop urinary diversion.

p.s. I just read in the op note that the patient did still have tubes and ovaries but no uterus and they also did a vaginectomy.
