G0438 in 95 year old with dementia

Many elder Assited Living residents with dementia ome to my office, without family. How would we do a AWV on these patients? Obviouly the patients can't answer any questions (but we have up to date charts with PMH, chonic cnditions, etc). And how do I handle the personalized preventive screening tests portion?
