Subsequent hernia repair

Alright, I need to pick some general surgery brains. Dr repaired an incisional hernia, with mesh. Five days later, the sutures holding the mesh dislodged from the fascia. He opened her back up and trimmed the mesh, and resutured it (one section, not the entire thing). There was obstructed bowel in this opening that he had to reduce back into the abdomen.

So, for the second procedure...any ideas? Internal hernia repair? Incisional (incarcerated) hernia again?
