What level of History should be given for ROS when patient is not able to respond?

ROS cannot be obtained
The clinician must document the history from the caregivers, nursing staff, family, EMS, etc. Also, document PMH as available from previous records. Then, the clinician may document that he/she is unable to obtain a ROS or remainder of the PFSH because....... State the reason.
If the clinician does that, you can credit a comprehensive history
