DOCUMENTATION GUIDELINES FOR SNFs

I have searched high and low for something that documents very specifically that the provider must be present on the DOD, in the facility, in order to bill either a 99315 or 99316. Although included in the codes, "...as appropriate, final exam of the patient, discussion of the stay..." this is not clear. As coders, we know it is required, however, the providers feel "it is impossible given that they have so many patients and facilities to visit". NO!! Would someone please send me in the right direction? Thank you!

Report the discharge service on the day the provider sees the patient, not on the date of discharge.
