Care Management/Care coordination of Registered Nurses(RN) and Social workers - Billing

Hey friends, I need to know how to bill for a RN,Social workers for their Care Management and Care co-ordination for chronic care ..Do they have to be under the provider group? Or they can form a group separately and go do the care management in hospitals , cancer care center etc.. I am not talking about TCM codes out here and I am aiming for codes 99487,488 and ..99489.. pls answer or guide me the link.. TIA

Can we bill commercials?

Can we bill all insurances, if it is fine..?

These services are typically only billable, especially for Medicare, when done by physicians or otherwise qualified health professionals, such as PAs. Not for RNs.
