Mastectomy for gynecomastia 19300

Is this code for a total mastectomy only or should it be used for any amount of tissue removed for gynecomastia that would otherwise be billed with code 19120 or 19301?

Does not have to be a "total" mastectomy. The surgeon could just be removing "extraneous" tissue to bring to normal size...but diagnosis should be gynecomastia (in a male)
