Medicare Wellness Exams

We billed G0402, 81003 and 36415, Medicare denied 81003 and 36415 as non covered services because this is a routine exam or screening procedure done in conjuction with a routine exam. We have also had EKG's deny when billed with the new G code for the same reason. Is this right? Has anyone else had this problem or is just our office?
