How to bill EMG/NCV on the same day.

Medicare has recently starting denying our emg claims. I looked up the emg codes and have changed appropriately, but still denied. When billing EMG/NCV on the same day how exactly do you bill? Is it suppose to be 95885 with a 59 modifier and then the 95903, 95904, 95934. Also do you list the NCV codes on a seperate line meaning 4 units of 95903 listed 4 times? I tried 59 after the 95903 and 95904 and they no longer work. Help!!!
