Interpretation date versus testing date

I need some help from all you experts out there. We attended a seminar this week and are now very confused and could use some help please!!!

1. If an EEG is done at another location(hospital)on Monday and the hospital is billing the TC portion and we (our doctor) read the EEG (in our office) on Wednesday and are billing the PC(26) portion only, what date do we report the interp, the date the test was done or the date we read the test?

2. Also if we read the EEG (in our office) do we list office as the POS or do we use the POS where the test was done?

3. In addition if the test was done in Ohio (which is CGS Medicare) and again we do the interp in Indiana (which is WPS Medicare) do we bill Ohio Medicare with POS Ohio hospital or do we bill IN Medicare with POS office?

4. We also do our own EEG's in our office and bill global for both the TC and PC portion. So again if the test was done on Monday and the physician didn't read it till Wednesday, what date would use as our billing date? The date the test was done or wait till the read was done to show you did all parts of the CPT® code?

5. Would any of your answers vary depending on if it's a Medicare patient versus a commercial insurance?
