Major & Minor Procedure

Hello,

I billed an E/M visit with 2 major procedure (90 days global) and 1 minor (10 day global). We billed the E/M with a 57 modifier to medicare, they denied payment on the E/M as billed within the global period of the minor surgery. My question is can you bill the E/M using both the 57 (major surgery/decision for surgery) modifier and the 25 (minor surgery) together?
