The ICD9 diagnosis code would probably be for drug abuse, unspecified - 304.9 - and/or history of drug abuse - 305.90-305.93.

The CPT® code might be 99408 or 99409 if you're documenting an initial screening with a tool like DAST, depending on face-to-face time.
