cerumen removal

For Humana insurnace we have been doing an office visit to go over whate the patient needs and some patients need a cerumen removal, at the same time. we use instruments to remove wax if we need to and when we do i bill for the office vist and the removal. how do i code this for humana they keep saying it is inculded. but it is not if that was not the reason for the visit and we use an instrument to remove the cerumen.
