Split billing

We recently added a NP to our practice and I've never really done split billing before. So here is an example of an office visit that I'm second guessing myself on:

NP saw patient, did a UA and KUB:
99204 - 25
81003
74000

Physician saw patient same day and did an in office cystoscopy:
52000

Right now I have them on two separate claims and listed as above under the providers as above. Does that look right?

Looks correct to me. Assuming NP has own provider number.
