What is the correct way to bill fluoroscopy guidance 77003?

77003 deals with the lumbar region.
ex. 62270 is billed with the 77003

Make sure that you check the ins. carrier and see what their guidelines are. For 2015 many procedures now include fluoroscopic guidance ex. 62311 and 62319, fluoroscopy is no longer billed separately with Medicare. So many carriers will be following suit.
